Report of Completion of Culminating Experience / Report of Completion for EdD Program / Certification of Approval

Once your Culminating Experience has been completed and approved, your committee must sign the appropriate documentation, which may include the Report of Completion for the EdD Program (for EdD students only), the Report of Completion Form, and/or the Certification of Approval (required for submitting a Thesis, Dissertation, or written Creative Work to the library).

Submitting Adobe Forms for Approval

To successfully submit Adobe forms for approval, you may need the following information: 
(Required information varies by form.)

1. Student Information

  • Name
  • Student ID Number
  • Degree Objective (e.g., Master of Science in Biology)
  • Email Address (Before submitting the form, students must verify their email address as the final step)

2. Approver Information

(e.g., Adviser, Department Coordinator, Department Chair, or Culminating Experience Committee Members)

  • First and Last Name
  • SFSU Email Address or a professional email address for committee members outside the institution

Submission Instructions

  1. Navigate to the appropriate form (see links below).
  2. Complete all required fields and select Submit.
  3. Enter the name and email address for each approver.
  4. Select Submit again.
  5. You will receive an email from Adobe Sign asking you to confirm your email address.
    • Click the confirmation link.
    • Once your email is verified, approvers will automatically receive the form for review and signature.
